PEANUT ALLERGY or TOLERANCE?

Basophil Activation Test

Utility: Measure the ability of basophils to respond to peanut stimulation

• Very high positive and negative predictive value

• Discriminate between peanut allergy and tolerance

• Reflect the severity and threshold of allergic reactions

• Useful to monitor and assess desensitization vs true tolerance

Clinical Indications/Descriptions:

The majority of IgE-sensitized children are not allergic. Furthermore, a considerable proportion of allergic patients have equivocal allergy test results. Oral food challenges to diagnose food allergies carry the risk of immediate allergic reactions, are resourceintensive, are in increasing demand, require multiple challenges per patient, and are not fool-proof. Up to 50% of the peanut challenges are positive. Specimen stability is 48 hours after venipuncture.

Assay Design:

Assay depends on identification of basophils, histamine release and B-regulatory cells.

Control                      Positive control (α-IgE)            Peanut allergen

  • Specimen:  4 ml peripheral blood in sodium heparin (green top)

  • CPT codes: 88184 x1, 88185 x30, 88188, 86353x2

Basophil activation test discriminates between allergy and tolerance in peanut-sensitized children (click to read more.)

“BAT (Basophil Activation Test) was able to distinguish between peanut sensitized from truly peanut allergic patients with almost 100% accuracy. It also had better diagnostic accuracy than peanut skin and serum IgE tests, when used as a standalone test. BAT could reduce the need for oral food challenges."
